JOB SUMMARY: Provides appropriate and clinically indicated services to veterans. Maintains clinical records within all federal, state, and city regulations documenting all services provided.
ESSENTIAL TASKS: – Completes clinical assessments for Veterans referred from the Department of Homeless Services (DHS). – Provides direct clinical services including individual, group, and crisis intervention as clinically indicated, including (but not limited to): outreach, assessment, evaluation, treatment planning, group and individual clinical psychoeducation and psychotherapy, skills development, crisis intervention, discharge planning, and referral. – Work closely with other community providers, including (but not limited to): psychopharmacology, case management, probation and parole, emergency services, health services, and Adult Protective Services to insure that clients receive comprehensive, coordinated services and treatment. – Collaborates with an interdisciplinary team. Participate in clinical case conferences and treatment planning meetings. – Documents service provision and maintain clinical records in a timely manner in accordance with ICL policies and guidelines, and state and local governmental entities, and CARF accreditation standards. – Complies with managed care requirements including consulting with care coordinators, obtaining treatment authorization, and providing necessary documentation. – Provides services in accordance with ethics, expertise and standards as described in the ICL policy on employee ethics and as defined by the applicable professional ethics-setting group (NASW, e.g.). – Maintains the highest level of professional competence through attendance and participation in staff development training activities – Attends weekly individual supervision with Director of Social Services. – Participate in program development and refinement. – Performs other duties as required
MINIMUM QUALIFICATIONS: Master’s Degree in Social Work from an accredited institution of higher learning and current, valid license (LMSW or LCSW) issued by the New York State Education Department. Maintenance of licensure/certification is a requirement to maintain employment in this position.
