Job title: Social Assistance Case Coordinator – Hostels/Homemakers & Nursing

Job description: Reporting to the Supervisor, Hostel Programs, is responsible for implementing and delivering financial and support services Housing with Supports, the Emergency Housing Program or Homemakers and Nursing Program (HNSA) to York Region clients; assessing the eligibility of applicants for financial assistance through Ontario Works; verifying the client’s information as required by the Ontario Disability Support Program (ODSP), government or private pension; and counselling and directing the resident to appropriate training, employment, health, housing, social services and community services programs.
Major Responsibilities
- Interviews applicants to assess and determine their eligibility income supports such as Homemakers and Nursing Assistance or assistance under the Ontario Works Act in Housing with Supports or the Emergency Housing or for per diem subsidy and Personal Needs Allowance (PNA); reviews documentation provided and verifies information for applicants.
- Determines allowance amounts and benefits available to the eligible applicants.
- Approves and issues benefits and authorizes disbursements and expenditures made to the resident or on behalf of the resident.
- Provides case management through interviews, screening, assessment, service planning activities, referrals to appropriate employment or community resources and follow-up.
- Manages resident’s financial eligibility through a structured monthly income review and makes adjustments to resident’s eligibility as circumstances change; follows up on concerns and complaints related to eligibility.
- Initiates referrals to mental health agencies, hospitals and other professionals and community agencies in cooperation with the Supervisor and Hostel Operators, as required.
- Maintains files and provides case administration in compliance with the appropriate Acts and regulations, such as the Ontario Works Act or Disability Support Program Act, Homemakers and Nursing requirements through the Ministry of Health and Long Term Care, Regional and/or York Region Domiciliary Hostel policies and procedures where applicable.
- Responds to inquiries about services, programs and incident reports and follows up, as required with appropriate compliance contact with the Licensing and Compliance Officer by completing a Hostel Referral Form.
- Performs other duties as assigned, in accordance with Branch and Department objectives.
Qualifications
- Successful completion of a Community College Diploma Program in Social Work, Social Science, or a related program.
- One to three (1-3) years experience in a social service environment, including one (1) year of direct Ontario Works case worker experience and experience with disadvantaged individuals, homelessness, mental health and senior population, employment counselling and group/workshop delivery and demonstrated skills to work in a demanding environment in emergency and domiciliary hostels.
- Valid Ontario Class “G” Driver’s License and reliable vehicle for use on corporate business.
- Satisfactory Police Criminal Background Check.
- Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
