It has one of the largest hospital-based research programs in Canada, with major research in transplantation, surgical innovation, cardiology, infectious diseases, genomic medicine and liver disease. .
POSITION:
The PHARMACIST provides comprehensive pharmacy services to patients in nephrology, with cross coverage as needed. As a member of the patient care team, the principal responsibilities include: conducting best possible medication history (BPMH) interviews, performing medication reconciliation, review all doctors’ medication orders for appropriateness; identify, resolve and prevent drug therapy problems with the patient and health care team; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines; follow-up on medication incidents and promote safe medication practices on the unit; respond to drug information questions; patient education. In addition, you will facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of patient needs come first, safety, compassion, teamwork, integrity and stewardship.
QUALIFICATIONS:
Education:
- Possess a Bachelor’s of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent required.
- Certificate of competency from the Pharmacy Examination Board of Canada required.
- Active registration with the Ontario College of Pharmacists (Part A) required.
Experience:
- Hospital pharmacy practice experience with patient-centered care experience in nephrology preferred
- Hospital Pharmacy residency an asset.
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset.

- Excellent organizational and time management skills.
