Job title: Patient Care Coordinator – Pharmacy

Job description: Responsible for communication with patients to ensure compliance with their prescribed therapies. Coordinates ongoing services for continued therapy. Acts as liaison for patient within varied service entities in Specialty Pharmacy and also with patient’s health care providers. Performs clinical assessment triaging as appropriate to pharmacist or healthcare provider. The role is characterized by a high level of responsibility through independent performance and problem solving under minimal supervision.
Qualifications
High school diploma or equivalent is required. Completion of an approved vocational Pharmacy Technician training program and at least eighteen months of applicable experience as a Pharmacy Technician; or at least three years of applicable experience as a Pharmacy Technician. Effectively reads, writes, speaks and understands English. Proven history of effectively performing technical tasks and following written/verbal instructions. Possess strong telephone and verbal communication skills. Experience with the use of office software computer applications and keyboarding. Demonstrated ability to work in a busy environment and pay attention to detail. Proven organization, problem solving skills, and exercise good judgement. Proven ability to effectively work both independently and in a team environment. Minimum 1 year customer service experience required. Background in a call center experience beneficial.
Additional qualifications
Prior experience performing advanced pharmacy technician activities preferred. Must possess excellent interpersonal skills to interact with patients and colleagues, including the ability to work collaboratively and professionally with people at all levels of education. Must possess excellent communication skills, both verbal and written. No history of drug abuse or misuse; or proof of successful completion of a certified drug abuse rehabilitation program is required.
License or certification
Current registration with the Board of Pharmacy in the state he/she is hired to work in where required by state regulation, or eligible to obtain. May not perform duties requiring registration until registration is obtained. Current National Pharmacy Technician Certification required.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $18.76 per hour.
