Job description: Serve as a Community Education Team(Comm-ED) Program member to assist with developing, implementing, and evaluating the Diabetes Education Program. Educator will be responsible for promoting program objectives in Community Ed areas, emphasizing diabetes education and diabetes self-management in both clinical and community settings in Suffolk County. The main objective is to provide prevention-focused and research-informed diabetes education to meet Suffolk County residents’ and professionals’ diverse needs. The position is responsible for using current research-based information from the ADA Clinical Practice Recommendations and the National Standards for Educational Programs to enhance the diabetes self-management practices of those who live with or work with pre-diabetes or diabetes optimize health outcomes. This individual represents Cornell Cooperative Extension’s family health and wellness programs to the public, community leaders, government officials, and Cornell University as part of program delivery. This position is funded by a contract with the Suffolk County Department of Health Services.

Required Qualifications:

  • Master’s Degree with experience relevant to the position. Or a Bachelor’s Degree plus 4 years of transferrable relevant evidence-based research experience may substitute.
  • Degree in Nursing (RN) or Dietetics (RD).
  • Experience in diabetes education, community outreach, and health care.
  • Ability to communicate effectively using a digital platform and oral, written, and visual channels using traditional tools and methods and electronic technology (e.g., video, audio, computers, etc.).
  • Demonstrated strong customer service skills.
  • Ability to follow procedures and written instructions.
  • Must demonstrate a high degree of integrity, sound professional judgment, and the ability to handle confidential and sensitive information.
  • Proficient with windows-based software, including the use of the internet and Microsoft Office Suite, including Outlook, Word, PowerPoint, Publisher, and Excel.
  • Ability to meet travel requirements associated with this position.
  • Ability to work flexible hours, which may include evenings and weekends, as appropriate.
  • Ability to effectively participate in professional team efforts.
  • Ability and willingness to work with diverse audiences and maintain cultural sensitivity.

Preferred Qualifications:

  • CDE (Certified Diabetes Educator) credential.
  • Experience with Cooperative Extension or other similar educational organizations
  • Ability to speak understand Spanish.

Responsibilities/Essential Functions:

Program Responsibilities:

Program Development – 5%

  • Assist supervisor with development of community educational programs to meet identified needs and diverse interests of participants.
  • Assist supervisor with development of Diabetes Self-Management and Healthy Lifestyles programming initiatives.
  • Assist supervisor in research, identification, and selection of existing resources.
  • Assist with identifying activities and resources necessary to implement initiatives and projects.
  • Assist supervisor in the creation of program materials to address identified needs.
  • Assist supervisor in making programmatic changes based on program evaluation outcomes and in meeting changing local priorities.
  • Assist supervisor in structuring programs to meet the needs of diverse program participants.

Program Delivery – 70%

  • Deliver high-quality Diabetes educational programs and events that address local, regional, and statewide programming priorities, emphasizing Diabetes Self-management and Healthy Lifestyles programming initiatives.
  • Utilize various educational methods that may include direct teaching, group experiences, media, newsletters, electronic technology, the internet, and distance learning to deliver the program.
  • Serve as a subject matter resource.
  • Foster acceptance of program, methods, and policies to address community and individual needs and consider diverse audiences.
  • Provide educational program efforts via mass media, including contributing articles to Family Health and Wellness Blog, utilizing emails, phone messages, and website postings.
  • Interact with program participants professionally when representing CCE.
  • Represent CCE before the public, community leaders, government officials, Cornell, or other land grant universities professionally as part of program delivery.

Program Evaluation – 5%

  • Assist in the creation of a program evaluation framework.
  • Interact with program participants, Association staff, program committees, county/community leaders, and other key stakeholders to collect evaluation data for improving programs.
  • Assist in analyzing collected program evaluation data for reporting and program improvement purposes using standard, established policy, procedure, and methods.
  • Assist with recommending program changes to strengthen and improve programs.
  • Communicate evaluation results and findings.

Administrative Responsibilities:

Direction – 5%

  • Assist with establishing Association program goals and priorities to align with Association and statewide Plan of Work.
  • Assist with planning marketing strategies, public affairs, and special events to reach target audiences and meet program objectives.
  • Assist in identifying community and fiscal resources for program continuance and enhancement.
  • Provide supervisor with program-related information to assist the budget development process.

Management – 5%

  • Assist in monitoring the program to ensure that educational content conforms to policies and objectives of the Association and is research-based.
  • Assist with monitoring program financial status to ensure compliance with funding requirements, program budget, grant specifications, and adherence to Association strategies/Plan of work.
  • Assist with identifying ways to reduce costs and maximize existing resources within the Association.
  • Make recommendations to the supervisor regarding program budgetary needs.
  • Assist supervisor in management of volunteers as needed.

Coordination/Operation – 5%

  • Maintain and prepare teaching materials necessary for Diabetes Education programs applying existing subject matter knowledge.
  • Maintain communication with constituents and the community through various methods such as newsletters, emails, electronic technology, the internet, social media, website postings, and content.
  • Assist with coordination of marketing and promotion of program through public relation initiatives and networking/collaborating with government, economic, educational, and other organizations.
  • Provide information as requested to assist with preparing reports, impact statements, and success stories on program progress and accomplishments for funders, advisory committees, Board of Directors, and others, as necessary.

Professional Improvement – 5%

  • Prepare for and actively participate in staff training and meetings.
  • Participate in staff evaluation processes as instructed.
  • Collaborate in activities that are in general support of Cornell Cooperative Extension and perform other duties as assigned.
  • Health and Safety – Applied to all duties and functions.
  • Support the Association to maintain a safe living and working environment.
  • Be familiar with and strive to follow any applicable federal, state, local regulations, Association health and safety policy/procedure/requirement and standard.
  • Act proactively to prevent accidents/injuries and communicate hazards to supervisors when identified.
