Job title: Counsellor Therapist 1 (Assessment Coordinator)

Job description: The Complex Care and Recovery Program at the Centre for Addiction and Mental Health (CAMH) is currently seeking an full-time, permanent Assessment Coordinator to work in the Adult Neurodevelopmental Services Outpatient.
Reporting to the senior manager, the successful candidate will have a key role in engaging patients, families and service providers, their service needs and planning the interprofessional team’s response to their referral. You will monitor and participate in processing referrals made to the program and in booking clients’ appointments in collaboration with the medical head and the team. You will have a capacity to participate in, and facilitate clinical team reviews collaborating with the team to determine and implement the next service steps based on the information collected. Following disposition decisions, you will support team members as they engage with patients and respond to the service request. You will communicate and liaise with the referral source as needed. You will monitor service wait-times to ensure adherence to the service timeline. You will liaise with other departments at CAMH and liaise/communicate with external stakeholders/partners with regard to patients’ referrals. You will perform other related activities as required. You will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. This position may require flexible hours, and it is located at 1001 Queen Street West.
Qualifications
The successful candidate will have a Bachelor’s degree in Social Work, Psychology, Nursing or a related health sciences field, combined with two (2) or more years’ clinical experience working directly with individuals with Autism, developmental disabilities and mental health needs. If in a regulated health care discipline, the candidate must show proof of registration and be a member in good standing within their respective College. The candidate will possess excellent organizational, communication and interpersonal skills in order to effectively manage complex relationships with community stakeholders in a self-directed manner and as a member of a dynamic interprofessional team. Experience working with community-based providers, providing effective leadership, chairing intake and clinical meetings; decision-making, organizational and problem solving skills are also essential. Hospital experience plus community based work experience with individuals with Autism, developmental disabilities, mental illness and/or addictions is essential. Experience collecting data for evaluation/research purposes is required. The ability to work with patients of diverse cultural and social backgrounds is required. Computer literacy is required. Familiarity with computer-based programs (MS Office, Outlook, and Excel) is essential. Bilingualism (French/English) or proficiency in a second language would be an asset.
Please Note: This full-time, permanent is part of the OPSEU bargaining unit.
Salary Range: $31.68 – $42.71 per hour
