Program Overview – First Step to Home, Housing & Homelessness Services
The First Step to Home (FSTH) team comprises health care professionals, social workers, and support staff who work collaboratively, within a harm reduction and recovery based mental health framework, to help individuals who are aged 55+ with substance misuse issues and/or mental health issues (concurrent disorders) make a successful transition from living on the street to a stable, healthier lifestyle. The individuals in the program are housed within WoodGreen Transitional Housing.
What You Will Do
- Provide comprehensive case management services, including but not limited to assessment, personal goal setting through the development of Coordinated Care Plans, short-term and long-term planning, clinical/supportive counselling, instrumental/practical assistance, follow-up supports, crisis intervention, service co-ordination, case conferencing, and referrals.
- Undertake psychosocial assessments and develop action plans with individuals in the program.
- Work closely with staff and clients on strategies that support clients with substance use and mental health issues to maintain their housing.
- Advocate on behalf of clients when required.
- Link clients to various community resources and facilitate appropriate referrals to other community support services when required.
- Undertake the role of Lead Care Coordinator (LCC), and actively participate in the Coordinated Care Planning process for organizing and managing ongoing communication in the care team.
- Ensure the coordination of services and a “continuum of care”.
- Liaise, consult, and collaborate with other program area staff and service providers.
- Monitor the housing challenges and ability of each resident to adhere to the policies and protocols of FSTH and develop a team strategy for intervention when problems occur in accordance with housing first and eviction prevention principles.
- Develop and lead psychotherapeutic and/or socio-recreational programming for clients as identified by the needs of clients and the program.
- Identify and implement community development and self-help opportunities.
- Provide mentorship, training, and support to placement students and volunteers.
- Actively participate in inter-agency initiatives aimed at identifying gaps in service and developing new and effective service strategies.
- Complete statistics/reports and maintain all records regarding program/activity enrollment.
- Ensure that the anti-racism objectives and community development principles of the organization are achieved in the program.
What You Bring to the Team
- Master Degree in Social Work (MSW) required.
- Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of two (2) years related experience in mental health and harm reduction.
- Experience working with older adults and/or seniors who have experienced homelessness.
- Demonstrated experience with housing help.

- Able to work independently with minimal supervision.
- Experience with mentoring and supporting placement students and volunteers.
- Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing needs.
- Proven ability to minimize barriers through service delivery for low income individuals with mental health and substance use issues.
- Excellent interpersonal, problem solving, multi-tasking, and organizational skills.
- Effective oral and written communication and documentation skills.
- Knowledge of and experience with a brief therapy modality, such as Narrative, Solution-Focused, Motivational Interviewing and/or Cognitive-Behavioral Therapy.
- Knowledge of mental health sector and services and resources available in the City of Toronto.
- Knowledge of the Mental Health Act, Residential Tenancy Act, and other relevant legislation.
- Knowledge of the Infection Prevention and Control Measures for Health Care Workers.
- Working knowledge and fluency in a second language is considered an asset.
- Demonstrated computer skills (Microsoft Office, Excel, Internet, databases).
