Program Overview – Counselling & Support Services, Community Care

The Counselling and Support Services team provides support for people with mental health and/or substance use issues, including concurrent disorders and dual diagnosis, within a trauma-informed, harm reduction, and mental health recovery framework.

What You Will Do

Direct Client Service:

  • Perform psychosocial and/or psychological assessments.
  • Develop and implement service plan and/or coordinated care plans.
  • Provide service/care co-ordination, system navigation, short-term and long-term planning, and follow-up support.
  • Provide preliminary assessment of risk in crisis situations, consult and devise an intervention and action plan, and provide crisis intervention.
  • Provide clinical and/or supportive counselling to clients utilizing a wide range of psychotherapeutic approaches that best meet the clients’ needs and circumstances.
  • Provide case management services to clients over an extended period of time, through the development and implementation of a care plan to address their priority and emerging needs.
  • Participate in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care.
  • Provide advocacy support and advocate with clients and/or their family or caregivers.
  • Link clients to various community resources, and facilitate appropriate referrals to other support services as required.

Psychosocial and Therapeutic Group:

  • Perform needs assessment to identify service needs and gaps that can be addressed through psychosocial and/or therapeutic groups.
  • In consultation with Manager/Director, develop psychosocial and/or therapeutic groups.
  • Develop/prepare group materials, including group curriculum, handouts, and supplies.
  • Facilitate therapeutic and/or psychosocial support groups, including DBT Skills group.
  • Perform outreach and coordinate with other agencies/providers to support provision of psychosocial and/or therapeutic groups.
  • Train and support development of co-facilitator(s).
  • Support evaluation and quality improvement measures for the groups.
  • Provide clinical support to individual group participants, as appropriate/required.

General Program:

  • Provide service that is responsive and culturally/linguistically sensitive to individual needs.
  • Uphold and consistently incorporate harm reduction, trauma-informed, anti-racism, and anti-oppressive principles.
  • Engage in ongoing outreach to ensure access to the program.
  • Keep current about the range of services available in other agencies.
  • Stay abreast of current clinical intervention approaches and best/promising practices.
  • Provide mentorship, training, and/or supervision to student placements and volunteers.
  • Maintain a client caseload as assigned by the Manager.
  • Provide services through home and office visits, emails, phone calls, etc.
  • Collect and report statistical data required for the accountability of the program.
  • Complete file documentation and data entry.
  • Represent the program/unit on internal staff committees as directed by Manager/Director.

What You Bring to the Team

  • Masters of Social Work (MSW) required.
  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required.
  • Three (3) years related experience in mental health, addictions, and/or social work.
  • Demonstrated experience facilitating therapeutic and psycho-social groups, including DBT-focused therapeutic groups is required.
  • Demonstrated experience supporting individuals from a harm reduction, trauma informed, mental health recovery, and anti-oppression approach.

What Will Set You Apart

  • Demonstrated ability in supporting individuals who have complex mental health and/or addiction issues, history of trauma, violence/abuse, chronic physical health issues, etc.
  • Proven working knowledge of theories of addiction, evidence-based treatment models, and best practice approaches.
  • Demonstrated ability in providing therapeutic interventions, such as Narrative Therapy, Solution-Focused Therapy, Cognitive-Behavioral Therapy, Dialectical Behavior Therapy, Motivational Interviewing, Trauma-Informed approaches, etc.
  • Experience providing individual and group-based addictions counselling and support.
  • Demonstrated experience in group facilitation and family/couples counselling.
  • Demonstrated ability in case management, system navigation, and service planning, implementation, and coordination.
  • Demonstrated ability in working effectively in a multi-disciplinary team or as a team member in collaboration with other professionals.
  • Strong assessment, clinical, and crisis intervention skills.
  • Effective oral and written communication and documentation skills.
  • Effective interpersonal, time management, and organizational skills.
  • Ability to be self-directed, independent, flexible, and exercise good judgment.
  • Ability to work from a client-centred approach.
  • Proven ability to minimize barriers through service delivery for low income individuals with mental health and/or substance use issues.
  • Comprehensive understanding and knowledge of the mental health, addictions, health sector, and community resources.
  • Demonstrate knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.
  • Applied computer skills (MS Office, Internet) and ability to use client information systems.
  • Ability to communicate in a second language is a strong asset.
