Position Overview – Social Work, Community Care

This position works on behalf of WoodGreen Community Services in the Assess and Restore (A&R)

partnership initiative with Salvation Army Toronto Grace Health Centre, St. Michael’s Hospital, and

Home and Community Care Support Services in the role of a Comprehensive Care and Integration Specialist (CCIS). The position will be primarily based out of Salvation Army Toronto Grace Health Centre and St. Michael’s Hospital. The A&R CCIS position is part of the Comprehensive Care and Integration Specialist (CCIS) team and works closely with Ontario Health and CCIS partners. The WoodGreen CCIS lead provides direction to the A&R CCIS around the work related to the CCIS team.

What You Will Do

  • Undertake psychosocial assessments.
  • Provide supportive and/or therapeutic counseling.
  • Work with hospital case managers, social workers and clinical staff, family doctors and others, to support identification of appropriate candidates for Assess and Restore (A&R); Make RM&R referral to rehab for candidates identified in the community; Work with primary care provider and client/patient to support direct referrals to the A&R program.
  • Act as a dedicated support to client/patients with complex health needs identified for A&R; Work with staff and the client/patient (and caregiver) while the client/patient is in hospital or rehab to identify the supports needed to enable a safe return to community.
  • Ensure seamless care by following the client/patient through all care transitions; Provide support until the client/patient/client is safely re-established in the community and any needed ongoing supports are in place.
  • Facilitate coordinated care plans based on the client/patients’ goals for their health care.
  • Provide the coordination of services and a “continuum of care” including attachment to primary care when needed; Participate in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care; Work closely with the patient/client (and caregiver) to ensure that the circle of care is working in an integrated way regardless of the number of providers and organizations involved.
  • Provide short-term navigation and intensive case management to support a successful discharge back to the community.
  • Work with the client/patient (and caregiver) once they have returned to the community to identify and fill any gaps in care that jeopardize their ability to live in the community or impede the care needed to avoid unnecessary returns to hospital or admission to Long-Term Care.
  • Link clients and/or their family or caregivers to various community resources.
  • Work with the circle of care to facilitate direct return to rehab when warranted that avoids patient/client going to the emergency department or an acute care admission.
  • Provide mentorship, training and/or supervision to student placements and volunteers.

What You Bring to the Team

  • Masters of Social Work (MSW) required.
  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Three (3) years of experience in mental health, harm reduction, geriatrics and/or social work.
  • Demonstrated experience working with older adults and seniors with mental health (including dementia) and/or substance use issues, chronic health issues, cognitive impairment, and responsive behaviours.

  • Proven ability to apply and practice therapeutic approaches and Harm Reduction strategies.
  • Ability to be self-directed, independent, flexible and exercise good judgment.
  • Ability to work from a client-centred approach.
  • Knowledge of and experience with a brief therapy modality, such as Narrative, Solution-Focused, Cognitive-Behavioural Therapy, Motivational Interviewing and Trauma-Informed approaches, Gentle Persuasive Approach, PIECES, Problem Solving Technique (PST), etc.
  • Knowledge of issues affecting older adults and geriatric populations and comprehensive understanding of the mental health, addictions, health sector, and community resources.
  • Ability to communicate in a second language is a strong asset.

Expected salary: $58379 per year
