Salary Range: $38.81 – $48.51 per hour
Hours: 37.5 hours per week weekdays
Status: Permanent Full-time

POSITION:
In the provision of patient and non-patient care activities, the primary responsibilities of the SOCIAL WORKER in the Musculoskeletal Rehab Program include: assisting MSK patients with psychosocial challenges and to optimize client progress in rehabilitation through the continuum; provision of comprehensive psychosocial assessments and intervention; provision of educational and supportive counselling services; provision of service, based on individual, family and group practice models; discharge planning, population health advocacy, community services linkage; collaboration the with inter-professional team and community based service providers; advocacy, mediation and conflict resolution; participation in professional, program and hospital development; willingness to provide field practicum instruction to Social Work students; participation in current and future program development, quality initiatives and research activities; compliance with, and commitment to, patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients; performing activities required for the effective and efficient operation / maintenance of the Social Work professional group; performing other duties consistent with the job classification, as required.
QUALIFICATIONS:
- Completion of a Master’s Degree in Social Work (M.S.W) or equivalent
- Registration with the Ontario College of Social Workers and Social Service Worker (OCSWSSW)
- Membership in the Ontario Association of Social Workers preferred
- Experience with populations of complex MSK, trauma and oncology
- Two years’ experience working with a multi-disciplinary team in a health care setting preferred
- Excellent verbal and written communications skills
- Excellent interpersonal, time management and organizational skills
- Excellent problem solving skills
- Research experience /education an asset
- Student education/supervision an asset
- Knowledge of policies, legislation, program’s and issues related to social welfare institutions and services
- Computer literacy preferred
- Knowledge of a second language an asset
