Program Overview – Caregiver Support & Wellness, Community Care

The Caregiver Support and Wellness team is responsible for enabling caregivers to thrive in their role. Through the provision of clinical social work services, this role will leverage exceptional skills in psycho-social assessments; time-limited/goal-oriented intensive case management; counselling; and system navigation to support caregivers who care for seniors living with geriatric, mental health, addictions, and cognitive issues such as dementia.

What You Will Do

  • Undertake psychosocial assessments and maintain a client caseload.
  • Develop/implement service plans/coordinated care plans with clients and their care recipients.
  • Provide service or care coordination, system navigation, short-term and long-term planning, and follow-up support and participate in case conferences and clinical meetings in order to ensure the coordination of services and a continuum of care.
  • Work closely with primary care and other service providers to support positive outcomes.
  • Work closely with other teams within WoodGreen to provide integrated/coordinated care.
  • Provide crisis intervention, practical assistance, and therapeutic and/or supportive counselling to caregivers in homes and community settings.
  • Advocate on behalf of clients and/or their family or caregivers.
  • Link clients and/or their caregivers to various community resources, and facilitate appropriate referrals to other community support services as required.
  • Facilitate or lead therapeutic/support groups.
  • Engage in ongoing outreach to engage stakeholders and build the presence of the program.
  • Maintain and develop contacts with community agencies and seniors’ groups and collaborate to organize joint health promotion clinics, workshops, and events.
  • Provide mentorship to social workers and training and supervision to volunteers and Bachelor and/or Master’s degree prepared students.
  • Develop relationships with other service providers in order to make seamless, effective referrals and service connections.
  • Provide service that is responsive, culturally, and linguistically sensitive to individual needs.
  • Maintain appropriate records and documentation in a timely manner.

What You Bring to the Team

  • Masters of Social Work (MSW) required.
  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Certificate in Gerontology considered an asset.
  • Minimum three (3) years of related experience in geriatrics and/or social work.
  • Demonstrated experience working with older adults and seniors with dementia, mental health and/or substance use issues, chronic health issues, cognitive impairment, and responsive behaviours is required.

  • Ability to work from a client-centred approach.
  • Proven ability to minimize barriers through service delivery for low income individuals with mental health and substance use issues.
  • Knowledge of and experience with a brief therapy modality, such as Narrative, Solution-Focused, Cognitive-Behavioural Therapy, Motivational Interviewing and Trauma-Informed approaches, Gentle Persuasive Approach, PIECES, Problem Solving Technique (PST) for caregivers, etc.
  • Thorough knowledge of issues affecting older adults and geriatric populations.
  • Comprehensive understanding and knowledge of the mental health, addictions, health sector, social determinants of health, and community resources.
  • Knowledge of the Mental Health Act and relevant legislation.
  • Applied computer skills (MS word, excel, Internet) and ability to use client information systems.
  • Ability to communicate in a second language (Greek, Spanish, Bengali, Urdu, Persian, Tagalog, Arabic, Mandarin, or Cantonese) is a strong asset.
