Full Time Position Location: Sioux Lookout, ON
Salary range starting from $69,093 to a maximum of $88,926/yearly
Diabetes has been identified as a high priority health issue impacting First Nations communities in the Sioux Lookout Zone. The Sioux Lookout First Nations Health Authority (SLFNHA) and the University of Toronto (U of T), have formed a partnership to improve community-based diabetes prevention, management, and care. A core element of our initiative is to enhance and expand a customized capacity building program for community health workers (CHWs), based on Canadian clinical guidelines, international best practices and local knowledge.
The CHW Diabetes Program Coordinator plays a key role in this initiative by leading and supporting the CHW Diabetes Program in participating First Nations communities. S/he will collaborate with the joint SLFNHA – U of T program team to undertake planning processes with community leaders and health system providers, support the implementation and enhancement of a training curriculum and education tools, and oversee the collection of monitoring and evaluation data.
The CHW Diabetes Program Coordinator reports directly to the Chief Operating Officer.

Applicants must possess:
- Education or training in nursing, health education, public health, primary health, community health or similar OR a combination of a post-secondary degree and relevant experience
- Diabetes Educator certification considered an asset
- Minimum 3-5 years of experience in coordinating / managing health sector projects or programs, including developing and implementing work plans and budgets
- Experience with monitoring and evaluation, community-based research and knowledge translation methodologies
- Experience working with First Nations communities in rural, remote and on-reserve settings and advanced knowledge of related community health issues considered strong assets
- Proven track record in developing professional relationships with different health system providers and community health leaders
- Excellent interpersonal and organizational skills including demonstrated ability to work productively in collaborative settings while also functioning independently
- Technical understanding of diabetes and other primary health issues and programming
- Strong oral and written (e.g. report writing) communication skills; ability to speak in a local dialect a strong asset
- Willingness to occasionally travel to remote Northern communities by car or small aircrafts
- Proficiency with MS Word, Excel, PowerPoint
Responsibilities:
a) Program Management and Implementation
Leads day-to-day program management and oversees implementation activities with a focus on operations and administration. Develops plans with the Program Team and local health leaders and providers. Manages and delivers internal program monitoring & reporting activities:
- Produce and implement detailed workplans and timelines and conduct quarterly reviews with Program Management Team
- Develop cost-effective budgets and manage funding within the approved budget
- Develop a communications plan to provide regular progress updates to the program team and external collaborators
- Ensure timely and effective monitoring of all program events and activities
- Provide routine narrative reports on program activities (e.g. progress against work plan)
- Report routinely on program finances and activity expenditure
- Report on program performance framework and related data collection
- Provide routine and ad hoc information for donor/grant reporting
- Oversee, support and advise on community engagement activity and training/capacity building program at participating sites
- Contribute to in-community engagement as required
- Contribute to broader SLFNHA/U of T program planning processes
- Chairs bi-weekly program meetings and quarterly team meetings
b) Human Resources
Manages program staff and resource planning:
- Recruit program staff and resources (e.g. develop job descriptions, conduct interviews)
- Lead staff on-boarding, orientation and capacity building
- Manage and mentor direct staff reports
- Contribute to the development of external consulting contracts
c) Representation
Act as a liaison and represent the program:
- Liaise and coordinate with community health leadership and local and regional health authorities/providers
- Organize and facilitate local forums and consultation processes
- Present program results at external meetings, forums and conferences
c) Research and Knowledge Translation (KT)
In collaboration with the program team, support program research and KT activities:
- Contribute to the design and planning of the program’s evaluation framework and related community-based research
- Develop a process for working within the research and ethics guidelines provided with the local research ethics board
- Provide logistical support for community-based research activities
Contribute to the development of methods and tools for sharing and disseminating knowledge generated by the program
