Job description: At Humber River Hospital, we are currently seeking a Manager, Integrated Transitions in Care. The primary focus of this role is to deliver care and support for areas of acute and ALC patient care with a focus on discharge planning, transitions in care, and psychosocial supports and to provide ongoing improvements for patient flow, quality outcomes, and operational performance in an efficient manner and in collaboration with community partners. The Manager will provide mobile assess and restore services to high risk frail older adults across inpatient and outpatient areas; to ensure high risk patients are followed from admission to discharge, and post-discharge, promoting continuity of care and consistent follow-up to better support patient transitions back to community, patient outcomes, and reduce avoidable readmissions.

Reporting Relationship: Program Director, Seniors Care

Hiring Salary Range: $50.609 – $53.772

Position Responsibilities:

  • Monitor and evaluate daily operational clinical performance to optimize the quality of patient care delivery in alignment with organizational goals and priorities
  • Support the development of the social work role to maximize the quality outcomes and experience for patients, families, staff and physicians.
  • Develop and monitor variances in unit operating budget in collaboration with the Director and Decision Support Services (DSS)
  • Coordinate discharge planning to ensure all patients are referred to the most appropriate services for their clinical needs, and proactively identify and resolve barriers to discharge for patients
  • Coordinate and redeploy social work to achieve appropriate coverage, departmental coverage needs, and associated risks
  • Manage daily clinical operations, and evaluate operational performance against department and organizational goals and priorities
  • Develop and implement policies and educational materials
  • Support the development of iPlan
  • Support initiatives in integrated care as identified by the Director.

Qualifications:

  • Regulated Health Care Professional in good standing with respective health professional regulatory body in Ontario
  • A graduate degree in a Clinical Discipline, Health or Business Administration required
  • Basic Cardiac Life Support (BLCS) Certification preferred
  • Membership with Nurses Association of Ontarion (RNAO) preferred
  • Certified Health Executive, preferred
  • Canadian Nursing Association (CAN) certificate in area of clinical focus, preferred
  • A minimum of (5) five years of clinical experience in an acute care hospital
  • A minimum of (3) three years management/leadership experience with direct people management responsibilities.
  • Proficiency in MS Office (Outlook, Word, Excel, Powerpoint, etc.)
  • Knowledge in Resource Matching & referral (RM&R) system
  • Knowledge of risk management and strategies, project coordination and management
  • Ameritech skills & knowledge, preferred
  • Analytical/reporting, program development and evaluation experience, preferred
  • Problem solving, leadership, relationship building and innovation experience
