Job Description
POSITION SUMMARY: Under the general supervision of the Director of Nursing, is responsible for coordinating best practices and quality improvement efforts including development, implementation, education, data collection and analysis related to documentation, technology, safety and security and clinical skill within the Home.
MAJOR RESPONSIBILITIES: * Implements and evaluates best practices and quality improvement initiatives as they apply to relationship centered care and nursing services including; clinical skills, documentation, safety and security, mandatory programs and customer satisfaction.
- Assists in the coordination, collection, analysis and reporting of the Homes Quality Improvement initiatives and mandatory reporting to Health Quality Ontario and the Local Health Integration Service
- Collects, conducts analysis and reports on data that includes standardized risk indicators, performance metrics, established bench marks, quality and risk activities and outcomes.
- Is knowledgeable and disseminates required information related to the homes, local and provincial initiatives including; Aging at Home, Resident First, and Behavioral Supports Ontario.
- In collaboration with the multidisciplinary team, researches, develops and regularly reviews programs and practices for the Section in order to meet legislated requirements, standards, and clinical best practices.
- Assists with tracking and trending Ministry of Health and Long-Term Care areas of non-compliance and assists by identifying areas to improve in order to meet Ministry compliance and/or improve consistency across the LTC and Seniors Services operations.
- Acts as liaison with the Electronic documentation software provider and Departmental contact in order to coordinate upgrades and technical improvements and provides technical support to the clinical staff.
- Assists with the development, coordination, and implementation of orientation for newly hired nursing staff.
- Acts as an effective change agent engaging and involving stakeholders at all levels of the Home; ensuring practices reflect changes in health care policy, technology, and the long-term care home environment. Effectively identifies, acknowledges, addresses and overcomes resistance to change as it occurs.
- Collaborates with the Director and the Section’s care team to update and develop policies and procedures related to clinical practice, delegated controlled acts, and added nursing skills.
- Performs such other related duties as may be assigned.
QUALIFICATIONS:
Education/Experience:
- Degree in nursing with a current unrestricted certificate of competence from the Ontario College of Nurses;
- Knowledge of CQI principles, practices and tools
Skills/Abilities:
- Must be at a basic skill level in word processing, spreadsheet, and presentation software;
- Ability to communicate effectively and concisely, both orally and in writing;
- Ability to analyze QI data and identify trends and corrective actions.
- Thorough understanding of clinical documentation systems as they apply to a long-term care setting;
- Good presentation skills and project planning skills;
- Ability to work independently without ongoing supervision;
- Must be competent within the meaning of the Ontario Occupational Health and Safety Act.
Assets:
- Post degree/diploma education in long-term care, statistics, and quality improvement
- Experience in Long Term Care or gerontology
CONDITIONS OF EMPLOYMENT:
- Must maintain certificate of registration – general class – from the College of Nurses of Ontario;
- Prior to commencing employment, will be required to review and attest/affirm to all Pioneer Ridge Staff Immunization Policies
- Must undergo a successful Police Vulnerable Sector Check (PVSC)
- Must have successfully undergone a “two step” tuberculosis skin test within the 12 months prior to employment
- Must be able to work flexible hours.
Expected salary: $33.81 per hour
