PROGRAM DESCRIPTION:

In January 2008, BRC launched its third new low-threshold transitional housing program, the Lex Safe Haven. Funded by the NYC Department of Homeless Services, the Lex Safe Haven is aimed at moving long-term homeless people from church steps and other public spaces throughout the city. Currently, this congregate housing facility provides housing for up to 43 couples and single men and women. Similar to other low-demand BRC facilities, case managers at the Lex Safe Haven are focused on moving chronically street homeless individuals into permanent housing. Program participants are not required to maintain sobriety and do not have any curfew; the one requirement upon entering is that they actively work toward obtaining housing.

HOURS:

Full-time, 37.5 hours per week

Monday – Friday; 7:00a – 3:30p

DUTIES/RESPONSIBILITIES:

Greet clients and referring outreach team. Conduct initial brief intake. Input and update information in AWARDS “Intake Note”. Enter clients into both AWARDS (Face Sheet) and DHS SCIMS (database). Enter Progress Note. Set new clients up with Nurse Practitioner for psych evaluation. Assist new clients without benefits with Homebound and/or Social Security applications. When needed escort clients to medical, entitlements, and housing related appointments. Assist Independent Living Specialist with HRA2010E Housing Application. Monitor and update all clients in DHS SCIMS and DHS CARES System. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

QUALIFICATIONS:

BA or experience working with homeless preferred. High School diploma or GED required. Knowledge of AWARDS preferred. Good writing and oral communication skills. Basic computer literacy required. Fire safety director strongly preferred. Must possess the ability to follow directions and work closely with others. CPR training certification or willingness to take training class in CPR.
