Job title: CH&E Coordinator (Community Mental Health Services Coordinator)

Job description: The Community Mental Health Services (CMHS) within the Complex Care and Recovery Program is currently looking for a full-time, permanent CH&E Coordinator (Community Mental Health Services Coordinator). You will work with hospitals to provide services that support the development, implementation, maintenance, and monitoring of Community Treatment Orders (CTOs) in Toronto. You will also work directly with clients, family members, substitute decision makers (if applicable) and community stakeholders in conjunction with inpatient staff to coordinate the implementation of CTOs as indicated. Your responsibilities will include: community development; education; facilitating ongoing communication between stakeholders during the development, implementation, and monitoring of CTOs. You will liaise between hospitals (emergency/inpatient), clients, family members, rights advisers, and community service providers throughout the CTO process. You will participate in the development of comprehensive CTOs that reflect client goals, detailed psychiatric follow-up, and access necessary supports for successful community living. You will maintain necessary statistical data and performing various related administrative duties. You will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. This position will require flexibility in work hours with some evening hours. This position is located in the Humber river regional hospital, but you could be required to travel between hospitals in the Greater Toronto Area (GTA).
Qualifications:
The successful candidate is a regulated health professional combined with two (2) years of experience working with clients with severe and persistent mental illness. A Master’s of Social Work, Master’s of Occupational Therapy, or a Bachelor of Science Degree in Nursing is preferred. As a regulated health professional, you will show proof of registration and be a member in good standing with your College. You have experience working with hospitals and community agencies and have detailed knowledge of the Mental Health Act and Health Care Consent Act. Although clinical skills are required, this position is highly administrative. Excellent interpersonal skills and strong written and verbal communication skills are required. You exhibit strong leadership skills, are self-motivated, and work successfully both independently as well as in team settings. Candidates proficient in a second language are preferred in order to meet the diversity needs of the various hospitals across the GTA. Bilingualism (French/English) is considered an asset. A valid driver’s license and access to a reliable vehicle would be an asset.
Please note: This full-time, permanent position is part of the OPSEU bargaining unit.
Salary Range: $33.01 – $44.53 per hour
